The Intake Interview
Onboarding a new infant in Title 22 order — from the parent's pre-work to a signed plan in the file, before day one.
No first day until the signed Needs & Services Plan and signed LIC 9227 are both in the child's file.
Start also waits on the health file — Physician's Report (LIC 701) with immunizations and Emergency Information (LIC 700). Owner: Director / Assistant Director (Amy).
The Pipeline
- Enrollment, package, and desired start date are verbally confirmed.
- Child, parent, and parent email (critical) entered into Brightwheel via the Admissions page.
- Admissions packet sent to the parent for fill-out — ① Feeding Plan (§101427), ② Onboarding Profile, and LIC 9227 (state sleep form, under-12-mo only, parent-signed in Brightwheel); ask for it back ≥48 hrs before the interview.
- Billing plan assigned and admission agreement sent.

- Walk the Google Form live with the parent — don't just send it to fill alone.
- Six sections: child & interview details · confirm pre-submitted forms · toilet training (if any) · services different from the normal program · routine / soothing / safety · review schedule & participation.
- Confirms each plan was submitted; covers feeding theory & honey (§101427(b)(5)).
- Deeper controls — each plan complete and signed — live in the checklist below, so the form stays short.
- Download the infant's Sleep Plan (LIC 9227) and Feeding Plan from Brightwheel and upload them into Claude first.
- After the interview, ask Claude to build the plan.
- The skill queries the linked response Sheet, pulls the child's row, and produces a compliant Needs & Services Plan as a Google Doc.
- Flags any next-review date set more than 90 days out, so the quarterly minimum (§101419.3) can't silently slip.
- Parent signs the plan to verify they helped build it (§101419.2(a)(2)).
- Google Docs eSignature or Brightwheel — whichever fits the family.
- Copy to the parent.
- Signed plan in the child's file.
- Give a copy to the infant team.
- Next review on the calendar.
Pre-start compliance checklist
- Feeding Plan is complete (8-point). All eight §101427 elements (A–H) are actually present on ① — not just that the form exists. Fix any gap with the family first.
- LIC 9227 is signed. The parent signed Section C — "submitted" isn't enough (§101419.2(b)(2)). Section D (provider-observed rolling) is completed later, when you observe it.
- Both signatures in the file. The signed N&S Plan and the signed LIC 9227 are both filed before the start date (§101419.2(a)). If either is missing, hold the start.
- Feeding-theory & honey talk. Captured on the form (Q26) — confirm it actually happened at the interview.
Sign, distribute, review
- Route for signature. Google Docs eSignature, or upload to Brightwheel and sign there — whichever fits the family. The parent must sign to verify they participated (§101419.2(a)(2)).
- Copy to the parent. Provide it and record the date on the plan (§101419.2(c)).
- Distribute to the room team. Routine, soothing, allergy, and special-service notes reach the lead teacher before day one.
- File the signed plan. In the child's record — Brightwheel plus physical file — available to Licensing (§101419.2(d)).
- Honor the review date. Title 22 requires updates at least quarterly. Re-run the form, regenerate the plan, and collect a fresh signature each time (§101419.3).
